About this site ...

We are pleased to offer continuing medical education to help Illinois primary care providers manage Illinois healthcare program patients. We understand your time is valuable, therefore we have developed easy-to-use CME activities that are timely and relevant to your profession. YHP’s CME activities can be viewed and completed through this Web site. Activities can also be downloaded to listen to at your convenience.

In addition, via IAFP’s CME Web siteexternal site link, you will be able to track which CME courses you have completed and obtain your CME certificates - simply with the click of a button at the end of each completed online course.

On this Web site you will find:
* Education – View slideshow presentations, download audio and other files, review documents and access QI tools
* Workflow Training Modules to help you efficiently and effectively implement quality improvement tools
* Clinical Training Modules to help you evaluate and manage common chronic conditions, such as pediatric asthma and adult depression
* Events – Schedules of training and user group meetings
* Forums – Post questions, participate in polls, and engage in discussions
* And, more! Answers to FAQs, how to apply for CME and additional links to other CME activities.

The workflow training modules on this Web site have been developed for and by primary health care providers. The modules will help you efficiently and effectively implement quality improvement tools, as well as evaluate and manage common chronic conditions, such as pediatric asthma and adult depression.

ABFM Credits for Diplomates

Currently two of the Your Healthcare Plus activities have been approved by the American Board of Family Medicine as an external provider of Part IV Maintenance of Certification for Family Physicians credit.

The Managing Adult Depression in Primary Care and Managing Childhood Asthma in Primary Care have been approved as quality improvement projects that would fulfill Part IV of the ABFM MoC. Approval for the other programs is pending.

To start a QI program first you must complete the one hour presentation on your selected topic and complete and pass the post test. After you have completed that you may start your QI project. There are detailed step by step instructions on what you need to start and complete a QI project in your practice. How to Complete an IAFP QI Project. Under the resources section you will find a Quality Improvement worksheet General QI worksheet and instructions on how to complete it How to Use QI worksheet. This worksheet will guide you through the QI process, and is also needed for documentation in order to receive CME credit and to receive MoC credit.

Program Overview

Your Healthcare Plus is a free benefit of the Illinois Department of Healthcare and Family Services designed to improve the quality of care and life for:
* Disabled adults with chronic or complex health issues
* Children and adults with asthma
* High frequency emergency room (ER) users

The focus of the program is on promoting and sustaining the patient-physician relationship. This voluntary service is provided to eligible Illinois healthcare program patients at no charge.

A multi-disciplinary care team, using evidence-based guidelines in support of the physician’s care plan:
* provide education and counseling to help patients establish and
use a medical home;
* help patients make informed health care decisions;
* improve patients’ understanding of their chronic diseases through self-learning
and management tools; and
* help patients’ receive care coordination for complex health needs.

Program Goals

* Enhance the health and wellness of patients.
* Work with providers to develop their program.
* Reduce avoidable inpatient and ER use.
* Increase the use of a medical home.
* Promote adherence to drug regimens that are cost and clinically effective.
* Increase the number of patients who receive care consistent with specific elements of evidence-based practice guidelines.
* Provide technology and office support that allows better communication across all team members.
